The following materials are also required for application:

  • Submit this Application Form (or mail the printed version of this application form)
  • Insurance Requirement:

  • Application Fee - $150.00 U.S. (non-refundable):
    Payment Options: Credit Card payable to CSUDH Foundation or other methods through email discussion.
  • Financial Verification*:
    Scholarship or bank statement verifying a minimum of $12,000 U.S. dollars for studies ($12,000 US dollars for the Intensive English Program; $12,000 for one-semester and $25,000 for two-semester Study Abroad America Program)
    (* For those applicants wishing to receive I-20's to obtain F-1 Student Visas for full-time study only.) 
  • Submit the Privacy Form (Microsoft Word format) if applying as an agent on behalf of a student.
  • Any deferral requests will incur an additional non-refundable fee of $100.
  • Housing and transportation is the student's responsibility. We will try to assist and provide resources.
